ATHLETE/CHEERLEADER/PARENT
ACKNOWLEDMENT FORM

Pop Warner Football & Cheerleading is an integral part of an individual’s overall educational and growth program. An Athlete/Cheerleader’s school activities, curricular and extra-curricular activities on the playing field, must be congruent with Pop Warner Little Scholar’s stated goals and objectives established for the intellectual, physical, social and moral development of its members/participants. It is within this context that the following Acknowledgement Form (in addition to the PW National Contract Agreements) is presented and voluntarily accepted by me.

As an Athlete/Cheerleader/Parent, I understand that it is my responsibility and obligation to:

1. Place academic achievement is a high priority. Respect the Head Coach’s ability to choose to suspend a player/cheerleader from practices or game(s) should conduct or academics become an issue. Any player/cheerleader suspended from school will not be allowed to play and will be placed suspension from the team until said player/cheerleader is reinstated to school.
2. Tustin Pop Warner Football and Cheer Association enforce a ‘Zero Tolerance’ policy to eliminate unnecessary verbal or physical abuse by spectators of the many volunteer Referees, Coaches and Board Members. Too often, people forget the real reasons why they are on the field and support the league: to encourage and support the children. We must promote fair plan and good sportsmanship and teach our children how to play the game and to have fun while competing. It is our responsibility to teach them that there is more to the game than winning or losing. It is for these reasons TPWF&C has adopted and will enforce a ‘Zero Tolerance’ policy.
3. Respect the integrity and judgment of game officials and association officials both on and off the field. This will include complete compliance with judgments levied by TPWFC including but not limited to, suspensions either short term or long term. Failure of compliance with judgments by any parent/guardian (married, single, divorced or acquaintance) will result in the immediate suspension of the player/cheerleader.
4. Respect that TPWFC will not tolerate comments or constant complaining about the Referees’ calls or Coaching decisions either during or after a game. If Coaches and/or spectators cannot control their comments or behaviors, they will be forced to leave the game area and may be suspended for the remaining season (OEC Rules).
5. Respect that the Orange Empire Conference (OEC) is the administrative body that acts as our agent in many areas necessary for each league to run smoothly.
6. Exhibit fair play, sportsmanship and proper conduct on and off the playing field. Maintain a high level of safety awareness and good sportsmanship. Know and follow all stated and applicable athletic rules and regulations as they pertain to eligibility and sports participation.
7. Refrain from the use of profanity, inappropriate attire, offensive display of body art, vulgarity and other offensive language and gestures, before, during and after a game, practice, competition and/or all Pop Warner, TPWFC, OEC team activities.
8. Adhere to established rules and standards of the game to be played. This includes, but is not limited to any and all rules as set forth in any and/or all of the applicable materials which are published on the OEC website (www.popwarneroec.com).
9. Refrain from the use of alcohol, tobacco, illegal and non-prescriptive drugs. The possession and/or use of alcoholic beverages and/or drugs are strictly prohibited at all games or practices. Players, Cheerleaders and their Parents are fully aware of this and agree to abide by it. As a parent, not only will I personally avoid using alcoholic beverages before or during games, I will instruct my child and other family members that it is improper to do so per Pop Warner Football & Cheerleading rules and regulations.
10. Refrain from electronic mass-communication (emails, Facebook, Twitter, My Space and/or phone calls) to the members of the TPWFC association or assigned team members in efforts to discredit or undermine the Head Coach, Coaching Staff or support team. I will contact the Head Coach direct or the Association Athletic Director and/or Coaches Representative.
11. All members understands that the league operates solely on contributions towards our non-profit organization and donations, fees and contributions will be compliant with all Rules and polices set forth by the IRS, Pop Warner, OEC and TPWFC. It is understood that each member will be responsible for full participation with the current season League (TPWFC) Fundraiser Program (TBA). Said Fundraiser Program will allow for an Annual Fundraiser Buy-Out Option of $50.00 due at the time of initial registration and prior to August 1st of the current year.
12. All fines accessed to a member for any violation of Pop Warner, OEC and/or TPWFC rules will be automatically accessed to the Team Account. Compensation to the Team Account by said fined member will be the responsibility of the Head Coach.

I further acknowledge, fully understand and agree that a violation of any of the above (real, valid, proven, established, justified, disputed, explainable or not) is absolute and immediate justification and grounds for my immediate removal from the Pop Warner Little Scholars program. This removal is absolute and non-disputable by me, my parents/guardians, coaches, teammates and others. However, I understand that under certain limited circumstances (but certainly not all circumstances) that limited Rights to Appeal may or may not apply.
